which might not be ideal

signals he’s taken a step back
into living fully

I used to "fight" with our culture. I didn't know I was "fighting." I thought I was just sharing my opinions as to the problems with our culture. Gradually I "got the memo" that I'd been "fighting" with our culture by impliedly "judging" those aspects of our culture as "wrong." Now I know There's Nothing Wrong with Our Culture; There's Only the Reality of Our Culture. Ironically, giving up judging (aka fighting with) the aspects of our human cultures that I think can be improved, I'm more in balance to make efforts to work towards changing human cultures.
Ah, worry about surgery! Also applies to things like knees and hips; the common factor seems to be that the affliction has to be unbearable for the patient to consent to go under the knife. And then afterward: "Gee, if I had known how improved I would be, I would have done this YEARS ago!" 🙄
